Am I being a complete noob, or is there a special way to show GC caches also on the maps from the GCA home page? I have tried clicking on a state, doing a postcode search and anything else I can think, but the only caches that show on the map are GCA. All the FAQ says that both are available, but I cannot figure it out - anon or logged in, map or searches all I get is GCA ones. Last time I tried I recall seeing everything, but not now.

The front page (necessarily) drives Geocaching Australia caches. That's why the site exists. Depending on the contribution you have made to the site (finds) you will see different front page search options with more finds showing up the "Search for a Geocache" section which allows you to select Geocaching Australia or All. This avoid issues with new started being overwhelmed by all of the search options.
To get to these straight off, in the menu, look for "Find a Geocache" and from there you can search everything and anything selecting Geocaching Australia or All as appropriate.

http://geocaching.com.au/my/twinkle/ has the string "require_once" in there, which should be "require".

It might also be that the backgrounds at http://geocaching.com.au/my/twinkle/ have been lost...
I do recall it having a satellite image of Australia or the states in the background.

Yep, that seems to have worked. It fails most days
It runs alphabetically, so by the time it gets to NSW it's exhausted. I am loathe to let it run during the day as it's a very DB heavy query which affects other DB queries.
I might have to disable it until we get the new server in place.
